To obtain your driver record for only $24.95 plus tax, follow these three easy steps:

Click on one of the links below for your preferred document format. The state produced DR1 document will appear. Print out the form and provide all of the requested information.

Step 2

Complete the payment information section. The processing of your driver record will begin when DefensiveDriving.com receives your signed form and your payment information via fax.

Step 3

Once the form is completed, signed and payment information provided, please fax it to DefensiveDriving.com at (713) 488-4102 (fax). DefensiveDriving.com cannot process your request unless the form is signed.
